Cardiac Care Measures
In collaboration with the American Heart Association, ÃÛÌÒÖ±²¥ established standardized measures for Comprehensive Cardiac Center (CCC) Certification, which was launched in 2017. In the spring of 2018 a Technical Advisory Panel (TAP) met to review proposed existing cardiac performance measurement requirements for certified CCC organizations.

The following are Acute Heart Attack Ready (AHAR), Primary Heart Attack Centers (PHAC), and Comprehensive Heart Attack Center (CHAC) chart abstracted measures used by ÃÛÌÒÖ±²¥.
Acute Heart Attack Ready
- AHAR-1 ECG within 10 Minutes of Arrival
- AHAR-2 Arrival to Thrombolytics within 30 Minutes
- AHAR-3 Arrival to Transfer to PCI Center within 45 Minutes (Door In—Door Out: Referring Hospital)
- AHAR-4 EMS First Medical Contact (FMC) to PCI < 90 Minutes (when applicable)
Primary Heart Attack Centers (PHAC)
- PHAC-1 ECG within 10 Minutes of Arrival at This Receiving Center
- PHAC-2 Primary PCI ≤ 90 minutes
- PHAC-3 EMS First Medical Contact (FMC) to PCI < 90 minutes
- PHAC-4 Arrival at First Facility to Primary PCI < 120 Minutes
Comprehensive Heart Attack Center (CHAC)
- CHAC-1 ECG within 10 minutes of arrival at this receiving center
- CHAC-2 Primary PCI ≤ 90 minutes
- CHAC-3 EMS FMC to PCI ≤ 90 minutes
- CHAC-4 Arrival at first facility to primary PCI ≤ 12o minutes
- CHAC-5 ACE-Inhibitor or angiotensin receptor blocker (ARB) for LVSD at discharge
- CHAC-6 Cardiac rehabilitation patient referral from an inpatient setting
- CHAC-7 ACE inhibitor or ARB prescribed at discharge for NSTE-ACS
- CHAC-8 Cardiac rehabilitation patient referral from an inpatient setting for NSTE-ACS
- CHAC-9 Dual antiplatelet therapy prescribed at discharge (NSTEMI)

ÃÛÌÒÖ±²¥ has chosen standardized chart abstracted cardiac care performance measures for five of the available cardiac certification options. The measures assess care provided to heart attack and heart failure patients, in addition to a cadre of measures that evaluate care provided within additional domains of cardiac care.
